  Effective Sept. 30, 2009, Air Gap Certificates must be completed by a certified ABPA/ASSE backflow assembly tester. A Backflow Assembly Test and Maintenance Report shall be submitted along with the Air Gap Verification form at the time of permit application. We can no longer accept faxed copies after permit has been issued.
